If you see an In-Network provider for your care, your network provider will handle any pre-certification required by the Plan. If you see an Out-of-Network provider for your care, you must ensure that the pre-certification has been obtained as required by the Plan by contacting UMR at 866.494.4502 orĀ www.umr.com.
