Complete this form to authorize the Pension Fund to directly deposit your pension benefit into a checking or savings account of your choice.

Pension – Direct Deposit Authorization Form

The Pension Direct Deposit Authorization Form may be completed online using our Member Portal.

Complete this form to authorize the Pension Fund to deduct Union Dues from your pension benefit.

Pension – Dues Deduction Authorization Form

The Pension Dues Deduction Authorization Form may be completed online using our Member Portal.

Complete this form to have Federal Income Taxes withheld from your pension benefit.

W4-P Withholding Certificate for Pension or Annuity Payments

The W4-P may be completed online using our Member Portal.

Complete this form to change your St. Louis Pension Benefit beneficiary only if you are receiving a benefit from the Pension Trust Fund of St. Louis that entitles you to appoint a new beneficiary.

Pension Beneficiary Change Form – St. Louis Death Benefits After Retirement

The Pension Post-Retirement Beneficiary Change Form may be completed online using our Member Portal.

This form is for use by unmarried St. Louis Pension participants only. If you are married, in all cases your Eligible Spouse is your beneficiary in the event of your death.
