The Pension Dues Deduction Authorization Form may be completed online using our Member Portal.
Complete this form to have Federal Income Taxes withheld from your pension benefit.
W4-P Withholding Certificate for Pension or Annuity Payments
The W4-P may be completed online using our Member Portal.
Complete this form to assign a new Pension beneficiary or change your St. Louis Pension Benefit beneficiary.
If you are retired and drawing your St. Louis Pension benefit, you may only change your Pension beneficiary if you are receiving a benefit from the Pension Trust Fund of St. Louis that entitles you to appoint a new beneficiary.
