As you know, effective January 1, 2023, your medical and behavioral health network is moving to UnitedHealthcare (UHC)/UMR. This change affects our non-Medicare population only.

New medical insurance ID cards will be in the mail no later than December 29, 2022.

Should you need your new cards prior to receiving them in the mail and before our offices open after the New Year’s holidays, sample cards are included here for your reference and emergency use.

Please note that the network information for UnitedHealthcare Choice Plus Network and UMR is the only information changing on your new card. The highlighted information below is the critical information you would need, should you need to access your benefits prior to receiving your card.

To use your UnitedHealthcare Choice Plus Network coverage effective 1/1/2023, you will need:

Issuer ID: (80840) 911-39026-02

Group: 76-415833

Your Member ID: 371-[your current Member ID]

Your new Member ID will be the same for you and your covered dependents. For your UHC ID, you will use the numbers “371” prior to your current Member ID. Your current Member ID can be found on your current medical ID card.

Should you not receive your medical ID cards within two weeks, or by January 12, 2023, or if you need an updated card sooner due to an existing physician visit, please call our office at 877.232.3863, to request a new card.
