The number of credit hours you worked in the Contribution Quarter will be subtracted from 330 hours. This total will be multiplied by the current Journeyman health plan contribution rate specified in the labor agreement. The result is the payment amount due. If you work no hours\, you pay for the full 330 hours. DESCRIPTION:The Siteman Cancer Center at Barnes-Jewish Hospital and Washington University School of Medicine is offering mammograms to women age 40 and over. Mammograms are available at no cost to women covered under the health plan who qualify. Patients under the age of 40 must have a physician’s order. \n\nAvailable at no cost to women who qualify (are covered under the plan).\nCarpenters Health Plan allows for one baseline screening between ages 35-39 with a physician’s order\, then every year for women 40 and over.\nYour most recent screening mammogram should be 12 months or more prior to the current mammogram for insurance coverage reasons.\nYou will also need a photo id\, your insurance card\, and the name of your primary care physician.\n\nCall Siteman Cancer Center to schedule your appointment at 314.747.7222 or 800.600.3606\, select option 2. \n

Thank you. \nEligible participants: Your full quarterly payment is due one month prior to the Benefit Quarter in which you need coverage. Coverage for April – June 2026 is due March 1; however\, payments are accepted for 31 days\, or until March 31\, 2026. \nPayments accepted: Credit card*\, ACH**\, and Check\n*Credit card payments are ONLY accepted through the Member Portal.\n**ACH payments may be made using the Member Portal; however\, ACH may still be taken over the phone by fund office staff during office hours. \nCall Participant Services with questions at 877.232.3863\, option 1. \nOnly ONE “One Time Exception” late payment is permitted. Participants must complete the appropriate paperwork for consideration. \n\nMinimum and Difference payment calculations & self-payments\nAll payments are based on 330 credit hours. The number of credit hours you worked in the Contribution Quarter will be subtracted from 330 hours. This total will be multiplied by the current Journeyman health plan contribution rate specified in the labor agreement. The result is the payment amount due. If you work no hours\, you pay for the full 330 hours. You can view your hours worked and/or make a payment to extend your health coverage by logging into your Member Portal. \nMinimum and Difference payments due\nThe full quarterly payment is due one month prior to the Benefit Quarter.
